"How much does it cost?" is the first question almost every patient asks about spinal decompression and Softwave TRT.
It's a fair question. These are cash-pay therapies, prices vary widely from clinic to clinic, and most offices won't give you a number until you're sitting in the exam room. We'd rather be upfront. Below you'll see what these therapies typically cost elsewhere, exactly what a full protocol costs at Prestige Spinal Care, why we recommend the number of sessions we do, and how you can try your first visit for just $99.
What these therapies cost at other clinics
Per-visit prices rarely tell you what a complete course of care will cost. Here's how typical national pricing stacks up against our full protocols.

- Lower decompression prices usually mean fewer sessions. Many clinics quote 8 to 12 visits, roughly half the standard IDD Therapy program.
- Not every "decompression" table is the same. Traction tables and true IDD Therapy systems like the Accu-SPINA are very different machines (more on that below).
- Some low prices are bundled. Certain clinics fold decompression into broader programs that can run $3,000 to $6,000, which makes apples-to-apples comparisons difficult.
What a full protocol costs at Prestige Spinal Care
Paid in full, our 20-session spinal decompression protocol is $2,000 and our 10-session Softwave TRT protocol is $1,000. That's $100 per visit for either therapy, compared with our standard rate of $195 per visit.

- Softwave TRT at $100 per session is well below the typical $150 to $250 range. Our full 10-session protocol costs about what many clinics charge for six or seven visits.
- Our decompression protocol includes a full 20 sessions on the Accu-SPINA. That's the complete IDD Therapy program, not an abbreviated 8- or 12-visit package on a traction table.
- You don't have to do everything at once. Our Comprehensive Healing Protocol pairs decompression, Softwave TRT and chiropractic adjustments for the best overall results, but many patients start with one or two services. Even a partial plan can bring meaningful relief.
Why we recommend 20 decompression sessions
Twenty sessions isn't a number we picked to sell more visits. It's the standard IDD Therapy program recommended by the Accu-SPINA's manufacturer and used by IDD clinics around the world, typically delivered over six to eight weeks (IDD Therapy, Physiopedia).
- Discs heal slowly. Spinal discs have very little direct blood supply. Decompression works by repeatedly relieving pressure on the disc so it can draw in fluid and nutrients, and that process builds over weeks, not days.
- Feeling better isn't the same as being better. Many patients notice relief partway through care. Stopping early can leave the underlying disc problem only partly addressed, which raises the odds that pain returns.
- The research is built on full programs. The outcomes published for IDD Therapy come from patients who completed a structured course of treatment, not a handful of visits.
A cheaper 8-visit package may look like a bargain, but if it stops before the disc has had time to respond, it can end up costing you more.
Why the Accu-SPINA makes a difference
Average pain score, out of 10, before and after IDD Therapy on the Accu-SPINA in a 2026 study of patients with neck and low back disc bulges and herniations. No adverse events were reported.
The Accu-SPINA is an FDA-cleared powered traction system (510(k) K033231) that delivers IDD (Intervertebral Differential Dynamics) Therapy. Rather than pulling on the whole spine, it uses computer-controlled forces to target a specific spinal segment, with gentle oscillations that help relax the surrounding muscles so the disc space can actually open.
Pain and disability both dropped sharply
Pain fell from 5.8 to 1.5 out of 10, and disability scores fell by about two-thirds, with no adverse events.
Better results than standard care alone
Adding IDD to standard care gave greater pain relief, better function and reduced herniation thickness than standard care alone.
Comparable to exercise-based physical therapy
IDD Therapy produced clinical improvement comparable to an exercise-based physical therapy program.
Tracked out to two years
Followed patients achieving a 50% or greater drop in pain at 2 months and again at 2 years.
As with any treatment, results vary from person to person, and researchers continue to call for larger long-term trials. But the evidence consistently points the same direction: targeted, full-course IDD Therapy helps many people with disc-related pain find meaningful relief without surgery.
Traction tables vs. true spinal decompression
A traction table and the Accu-SPINA can look similar, but they work very differently, and the research behind them is not the same. A major Cochrane review of 32 trials found that traction, alone or with other treatments, provides little or no benefit for low back pain (Cochrane, 2013). IDD Therapy on the Accu-SPINA was designed to overcome traction's limitations.
| Feature | Traction tables | IDD Therapy on the Accu-SPINAWhat we use |
|---|---|---|
| Time on the table | Usually 12 to 15 minutes | 25 minutes per session |
| How force is applied | A general pull on the whole spine | Computer-controlled forces aimed at the specific disc level causing your pain, set by adjusting the angle of pull |
| Muscle guarding | The body tends to tighten against the pull, limiting how much the disc space opens | Gentle oscillations relax the surrounding muscles so the targeted segment can be decompressed |
| Typical course | 8 to 12 visits | 20-session standard program over 6 to 8 weeks |
| Evidence | Little or no benefit for low back pain in a review of 32 trials | Randomized trials and multi-year follow-up showing reduced pain and improved function |
Why session length matters. The extra time on the Accu-SPINA gives the muscles time to relax and the disc time to respond during each treatment. Shorter traction sessions often end before that can happen.
Why it lasts. Research on IDD Therapy has tracked patients well beyond the last visit. Its developer reported an average 65% drop in pain after treatment that improved to 76% at one year, and a separate study of 415 patients followed outcomes out to two years (Physiopedia). Combined with core strengthening and healthy movement habits, many patients keep their results long after the protocol ends.
Softwave TRT: what you're paying for
Softwave TRT uses the OrthoGold 100, a device FDA-cleared to temporarily increase local blood circulation, relieve minor aches and pains, and activate connective tissue (MedEsthetics, 2021).
Unlike many shockwave devices that focus energy on a single small point, Softwave delivers a broad, unfocused wave over a wider treatment area. It's noninvasive, needs no medication or downtime, and most sessions take only a few minutes. Shockwave therapy in general has a strong safety record, and its best-studied uses include plantar fasciitis and chronic tendon problems (Health Technology Wales).
Why 10 sessions? Most clinics quote 6 to 12 Softwave sessions for maximum relief. Our 10-session protocol sits in that range and gives the tissue repeated stimulation while it heals, at $100 per session instead of the $150 to $250 many clinics charge.
Paired with spinal decompression, Softwave helps calm inflamed soft tissue around the spine while decompression addresses the disc itself. That's why we combine them in our Comprehensive Healing Protocol.
Is it worth the cost?
Compared with surgery, a full non-surgical protocol is a fraction of the price, before you even count recovery time and lost work.
Surgery isn't the only cost to weigh. Ongoing back, neck or nerve pain has its own price tag:
- Months of co-pays, prescriptions, injections and over-the-counter pain relievers that manage symptoms without addressing the cause
- Missed work, lost productivity and turning down activities you love
- Poor sleep, less time on your feet with your family, and the stress of not knowing when the next flare-up will hit
We also make the investment manageable. Pay in full for the best value, split payments with our in-house PSC financing, or use CareCredit. Packages are valid for one year, so you can complete care on a schedule that fits your life.
